Foster Closet Application Form
The Foster Closet at Innovation seeks to provide foster families with clothing and basic care items to help foster families overcome the challenges associated with fostering a child.
If you are a foster family, relative placement or adoptive family you qualify to shop at the Foster Closet!
To be eligible to use the foster closet as a resource, required proof of relationship documents for children (ages newborn to 18) are as follows. Parents/guardians please bring copies of documents for each placement/child living in your home to your FIRST VISIT or when you receive a new child in your care.
You can come shop 2x's a year as the seasons change!
On your 1st Visit you will receive:
- 1 re-usable bag for each child to use for each time they come to shop
- a new package of underwear
- a new package of socks
- a welcome bag full of toiletry items
- 2 pairs of shoes
- 1 pair of pjs
- 5 outfits (5 tops/ 5 bottoms)
- as needed misc item
All remaining visits (every 6 months) you will receive:
- 2 pairs of shoes
- 1 pair of pjs
- 5 outfits (5 tops/ 5 bottoms)
- as needed misc item
